Clarification of originally certified documents

All copies of supporting documents such as probate, power of attorney, birth, death or marriage certificates etc. should be have an original certification.

An originally certified copy is a copy of an original document which has been certified as being a true copy by an authorised person. It should state the authorised person's name, title and address.

If you have changed your address, bank details, email, phone number or Tax file number, the following documents are to be sent to PO Box 30 Bendigo, Victoria 3552.

  1. Shareholder details form
  2. Certified copy of Identification. The documents must be an original certification.

Reissue of lost share certificate

The following documents are to be sent to P.O. Box 30 Bendigo, Victoria, 3552

1. Relevant certificate replacement form:

Certificate Replacement Request – Individual and Joint Holdings

Certificate Replacement Request – Executors/Administrators

Certificate Replacement Request – Companies

2. Certified copy of identification.

The documents must contain an original certification.

Change of shareholder name

The following is to be provided to P.O. Box 30 Bendigo, Victoria, 3552

1. Change of Name Request

2. Supporting document: evidence of name change, this can be a document from the Department of Births, Deaths & Marriages, drivers licence etc. The document must contain an original certification.

3. Please send your original share certificate or the relevant certificate replacement request in order to have a certificate issued in your new name.

Tax File Number (TFN), Australian Business Number (ABN) or Exemption Notification

Transfer of shares

The following documents are to be provided to P.O. Box 30 Bendigo, Victoria, 3552

1. Transfer form for non-market transactions

2. Original share certificate. If this cannot be located please complete the relevant certificate replacement request.

Certificate Replacement Request – Individual and Joint Holdings

Certificate Replacement Request – Executors/Administrators

Certificate Replacement Request – Companies

3. Certified copy of identification. The documents must contain an original certification.

4. Shareholder details form

Joint holding: Where the holding is in more than one name, all shareholders must sign.

Power of attorney: If a power of attorney has been nominated, attach a certified photocopy of the power of attorney to the transfer form.

All transfers are subject to board approval.

Transfer of shares to a super fund (self managed)

The following documents are to be provided to P.O. Box 30 Bendigo, Victoria, 3552

1. Transfer form for non-market transactions

2. Original share certificate. If this cannot be located please complete the relevant certificate replacement request.

3. Certified copy of identification. The documents must contain an original certification.

4. Shareholder details form

Joint holding: Where the holding is in more than one name, all shareholders must sign

Power of attorney: If a power of attorney has been nominated, attach a certified photocopy of the power of attorney to the transfer form.

All transfers are subject to board approval.

Deceased - joint holding

If one holder of a joint shareholding passes away, the shares can be transferred to the surviving shareholder. The following documents are to be provided to P.O. Box 30 Bendigo, Victoria, 3552

1. 'Request to Register Surviving Holders' form

2. Original share certificate(s) or if this cannot be located, a 'Certificate Replacement Request – Executors/Administrators'

3. Certified copy of death certificate: Certification Definition

4. Certified copy of identification. The documents must contain an original certification.

5. Shareholder Details Form

Deceased - Individual transfer to estate name

If the shares are to be sold, they need to first be placed in a state of "Transmission". The following documents are to be provided to P.O. Box 30 Bendigo, Victoria, 3552

1. 'Transmission Application' form

2. Original share certificate(s), or if this cannot be located, a 'Certificate Replacement Request – Executors/Administrators' form

3. Certified copy of death certificate

4. Either a) Certified copy of probate

OR both

a) Certified copy of the will

b) 'Small Estate Statement and Indemnity'

5. Certified copy of identification. The documents must contain an original certification.

6. Shareholder Details Form

An original certified copy is a copy of an original document which has been certified as being a true copy by an authorised person. It should state the authorised person's name, title and address.

Deceased - individual transfer to beneficiaries

If the shares are to be transferred directly to a beneficiary, the following documents are to be provided to P.O. Box 30 Bendigo, Victoria, 3552

1. 'Transfer form for non-market transactions' – one form per beneficiary

2. Original share certificate(s) or if this cannot be located, a 'Certificate Replacement Request – Executors/Administrators'

3. Certified copy of Death Certificate

4. Either a) Certified copy of probate

OR both

a) Certified copy of the Will

b) 'Small Estate Statement and Indemnity'

5. Certified copy of identification. The documents must contain an original certification.

6. Shareholder Details Form
